Ask the Expert: Identified Need

JCAHO Patient Safety Goal, an analysis of a patient fall in Imaging Services identified the need to improve communication among caregivers at Santa Rosa Memorial and Petaluma Valley Hospitals. During the analysis, Gary Greensweig, M.D., Medical Director of Imaging Services, Norman Shore, Director of Imaging Services, and department staff members identified that patient risks were not communicated when patients were transferred to their department. It was then that the idea for Ticket to Ride was created.  Whether a recent speeding ticket or the Beatles’ lyrics was the impetus, Norman collaborated with the Emergency Department, Nursing Units, and Transportation Department Gary and his staff members to develop the Ticket to Ride process. The policy was designed so that a patient does not leave the Nursing Unit or ED Department until the assigned nurse and transporter complete and sign the following information on the Ticket to Ride form:

Patient Name

Date of Birth, verified, patient location

Is the patient alert?

Is the patient oriented?

Does the patient need meds or has the patient recently been medicated?

Does the patient have infection control issues, such as MRSA, VRE, TB, etc.?

How does this patient transport?

Does this patient have spinal precautions or any other special needs?

Does this patient have multiple exams in Imaging today or waiting to be discharged?

Is there a DNR order?
