Ask the Expert: Risk Management Considerations

General risk management considerations  

  • Until such time as a protocol is agreed upon by the medical/dental community, use conservative approaches that best reflect current literature
  • Networking with medical colleagues and your specialty professional organizations to establish a pre-therapeutic regimen for patients requiring bisphosphonates
  • Initiation of bisphosphonate therapy should be pre-empted by a dental/maxillofacial assessment, whenever possible
  • Treatment of all invasive or potentially problematic dental conditions should be completed prior to aggressive bisphosphonate therapy
  • Conducting a thorough medical history, including a review of current and previous medical conditions as well as medications
  • Proceeding very cautiously with all surgically invasive procedures on patients who have received intravenous infusion of high doses of bisphosphonates over a prolonged period of time
  • Consulting with medical colleagues to obtain total dosage history and discuss potential patient risks
  • Carefully executing your informed consent discussion and documentation of all material risks, benefits and alternatives to treatment
  • Thoroughly document all findings, diagnoses, consultations, plans, treatment and rationale, as well as, post-operative  instructions and follow-up
